When to Use the Product Claim Form
You can submit a product claim for the following reasons:
- Warranty Claims: If your product is covered under warranty.
- Damaged Upon Arrival: If your product arrived with damage.
- Missing Parts: If specific parts of your product were not included in the package.
- Missing Products: If your order was incomplete and items are missing.
Information You Need to Provide
To submit your claim, please prepare the following details:
- Claim Type: Select the reason for your claim (Warranty, Damaged Upon Arrival, Missing Parts, Missing Products).
- Order Number: Your order confirmation number, which can be found in your order receipt or email.
- Customer Name: The name used to place the order.
- Phone Number: Your contact number in case we need further information.
- Email Address: Your email address for updates on your claim.
- Damaged Product (SKU): The SKU of the damaged or affected product.
- Damaged Part Number (Optional): If applicable, the specific part number of the damaged item.
- Shipping Address: Your address to verify delivery details or for possible replacements.
- Description of the Issue: Provide a detailed explanation of the problem with the product.
Attach Images
Images help us process your claim faster. Please include:
- For Missing Parts: Clear pictures of all the parts you received.
- For Damage: Clear pictures of the damaged part(s).

How to Submit the Form

- Fill Out the Form:

- Attach Photos:
